Job summary

Chapters Health System in Florida seeks an experienced RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to keep patients at home as long as possible.

The role requires a current Florida RN license, at least one year of hospice or hospital experience, EMR/EHR proficiency, and the ability to travel to patient homes. Strong communication and SBAR documentation are essential.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year nursing experience in hospice or hospital.
• Experience with an EMR/EHR system.
• Valid driver’s license and automobile insurance.
• Ability to visit participants in their homes.
• Proficient in English communication, both verbal and written.
• Active BLS certification for healthcare professionals.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patient, family and caregivers about disease, palliation, and safety.
• Home visits to assess safety, medications and community living viability.
• Communicates changes in condition to IDT and initiates appropriate plans.
• Supervises care provided by Home Health Aides as requested.
