Job summary

Chapters Health System in Florida is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and deliver clinical, palliative and supportive care to help patients remain at home as long as possible.

The role requires an active RN license, at least one year of nursing experience, experience with EMR/EHR, a valid driver’s license and the ability to travel to patient homes. BLS certification is also required.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinate the Plan of Care reflecting evolving patient needs.
• Educate patient, family, caregivers and health professionals about disease, palliation, and safety.
• Report changes in patient condition to IDT and adjust care plans.
• Participate in IDT meetings and document communications in the EMR.
• Supervise Home Health Aides as requested.
