Job summary

Chapters Health System seeks an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide compassionate palliative and supportive care in the home setting.

The role requires an active RN license, at least 1 year of nursing experience, EMR/EHR proficiency, and a valid driver’s license. Travel to patient homes and strong communication skills are essential, with BLS certification required.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants in their homes to conduct assessments.
• Ability to perform essential functions (lifting, bending, pushing/pulling) with/without accommodation.
• Active BLS for healthcare professionals from AHA or Red Cross.

Responsibilities

• Provides direct care to patients and families as part of IDT, with psychosocial, spiritual, cultural and physical components.
