Mayo Clinic is seeking an RN Case Manager to support care coordination across the continuum of care. You will help develop and communicate the patient care plan, coordinate with the interdisciplinary team and outside agencies, and lead case management education within Mayo Clinic’s practice model.

The role emphasizes leadership in communication, documentation, and utilization management, with TN sponsorship available for eligible candidates.

Qualifications

• Graduate of an accredited nursing program.
• RN license in the applicable state.
• One year of RN case management experience preferred.
• Certification (CCM or ACM-RN) preferred.

Responsibilities

• Facilitate the patient plan of care across the continuum of care.
• Coordinate care within an interdisciplinary team and with outside agencies.
• Lead education on case management/utilization concepts and participate in committees.
• Ensure high-quality care delivered in a cost-effective, timely manner.
