Job summary

Mayo Clinic in Phoenix/Scottsdale, AZ seeks a Registered Nurse Case Manager to coordinate patient care across the continuum, ensuring utilization management, care coordination, and clinical documentation within the Mayo Clinical Nursing Practice Model.

The RN Case Manager leads a multidisciplinary team, advocates for patients and families, and collaborates with providers and external agencies to deliver high-quality, cost-effective care.

Qualifications

• One year of RN Case Management experience or successful completion of the MCSHS RN Case Manager Fellowship within six months of hire.
• 3 years of acute nursing preferred; 1 year of Case Management experience preferred.
• Certification (CCM or ACM-RN) preferred.
• Current RN license by applicable state requirements; Arizona - Maintains BLS competency.
• Additional state licensure and/or specialty certification/training as required by the work area.

Responsibilities

• Coordinate the patient plan of care across the continuum with utilization management and care coordination.
• Provide leadership and education within the team; bridge communication between providers, patients, families and outside agencies.
• Promote patient/family advocacy to achieve quality clinical, financial and satisfaction outcomes.
• Ensure care progression is documented and aligned with nursing practice standards.
