Mayo Clinic in Phoenix/Scottsdale, Arizona is seeking a Registered Nurse Case Manager to coordinate the patient plan of care across the continuum, ensure utilization management, and lead in the interdisciplinary team.

This role emphasizes patient/family advocacy, education on case management concepts, and collaboration with providers and outside agencies to deliver high-quality, cost-effective care. An RN license, current BLS, and CCM/ACM-RN certification preferred; TN sponsorship available.

Qualifications

• Arizona-registered nursing program accreditation required for eligibility.
• If graduation from a non-accredited program, at least one year of RN experience is required.
• RN Case Management experience or completion of the MCSHS RN Case Manager Fellowship within six months of hire.
• 3 years of acute nursing preferred; 1 year of Case Management experience preferred.
• CCM or ACM-RN certification preferred; valid RN license required; BLS competency.

Responsibilities

• Coordinate patient plan of care within an interdisciplinary team.
• Ensure utilization management, care coordination, and clinical documentation.
• Provide leadership through education on case management concepts.
• Bridge communication between providers, patients, families, and outside agencies.
• Support research, committee work, and community involvement.
