Job summary

Mayo Clinic in Peoria, AZ is seeking a Registered Nurse Case Manager to coordinate patient care across the continuum and ensure appropriate utilization and documentation. You will lead education on case management concepts and collaborate with the interdisciplinary team to improve quality and efficiency.

The role requires RN licensure in Arizona, BLS, and prior case management or fellowship training; sponsorship for TN may be available.

Qualifications

• Graduate of an accredited nursing program recognized by ACEN/CCNE/NLN CNEA.
• If not ACEN/CCNE/NLN CNEA, at least 1 year RN experience in an applicable care setting.
• If graduation not within last 2 years, 1 year RN experience or Mayo Clinic RN Case Manager Fellowship.
• 3 years of acute nursing preferred; 1 year of Case Management preferred.
• Current RN license by applicable state requirements; AZ BLS competency.

Responsibilities

• Facilitate patient plan of care across the continuum through utilization management and care coordination.
• Provide leadership and education on case management concepts; participate in committees and quality initiatives.
• Bridge communication among providers, patients and families, as well as external agencies to ensure high-quality, cost-effective care.
