Job summary

Mayo Clinic in Arizona is seeking an RN Case Manager to collaborate with an interdisciplinary team and coordinate the patient plan of care across the continuum. The role emphasizes utilization management, care coordination, and timely documentation to achieve quality outcomes.

Qualified candidates will have current RN licensure, at least one year of RN or case management experience, and preferably CCM/ACM-RN certification. This full-time position offers competitive compensation and benefits.

Qualifications

• Graduate of accredited nursing program with RN license.
• One year RN experience if program not ACEN/CCNE/NLN CNEA.
• One year RN Case Management experience or completion of MCSHS RN Case Manager Fellowship within six months of hire.
• 3 years of acute nursing preferred; 1 year of Case Management experience preferred.
• Certification (CCM or ACM-RN) preferred.
• Arizona licensure and BLS competency.
• Additional state licensure/certifications as required by work area.

Responsibilities

• Coordinate patient care plan across the continuum to ensure appropriate utilization management.
• Provide leadership within the interdisciplinary team and facilitate communication with providers, patients, families and outside agencies.
• Educate on case management/utilization concepts, participate in committee work and research.
• Ensure documentation supports quality clinical, financial and patient satisfaction outcomes.
