Job summary

Mayo Clinic in Arizona is hiring a Registered Nurse (RN) Case Manager to support the patient plan of care across the continuum. You will help ensure appropriate utilization management, care coordination, and timely documentation within our interdisciplinary team.

You will lead initiatives, educate colleagues on case management concepts, and collaborate with internal partners and outside agencies to deliver high-quality, cost-effective care while maintaining Mayo Clinic standards.

Qualifications

• Arizona RN license and graduate of accredited nursing program.
• One year RN Case Management experience required.
• CCM/ACM-RN certification preferred.
• Basic Life Support (BLS) competency required.

Responsibilities

• Coordinate patient care plans across the continuum of care.
• Lead care coordination with interdisciplinary team.
• Educate colleagues on case management concepts and guidelines.
