Job summary

Mayo Clinic is seeking an RN Case Manager to work within an interdisciplinary team to coordinate patient care across the continuum. The role emphasizes utilization management, care coordination, and documentation, aligning with Mayo Clinic’s standards for quality, safety, and efficiency.

The position requires Arizona RN licensure, 1 year of Case Management experience (or completion of the fellowship), and preferred CCM/ACM-RN certification.

Qualifications

• One year of RN Case Management experience or completion of the MCSHS RN Case Manager Fellowship within six months of hire.
• 3 years of acute nursing preferred; 1 year of Case Management experience preferred.
• Certification (CCM or ACM-RN) preferred.

Responsibilities

• Facilitate patient plan of care across the continuum; ensure appropriate utilization management and care coordination.
• Lead and educate within the team to improve quality, financial, and patient satisfaction outcomes.
• Communicate with providers, patients, families, and external agencies to ensure timely, cost-effective care.
