Job summary

Mayo Clinic is seeking an RN Case Manager to work within an interdisciplinary team to coordinate patient care across the continuum, ensure appropriate utilization, and improve outcomes. This role supports the Mayo Clinical Nursing Practice Model and fosters patient advocacy and team leadership.

The position requires Arizona RN licensure, BLS competency, and may require CCM/ACM-RN certification; TN sponsorship is available for this role.

Qualifications

• Arizona RN license required.
• 1 year RN Case Management experience required or completed fellowship within 6 months of hire.
• CCM or ACM-RN certification preferred.

Responsibilities

• Facilitate patient care plan across continuum within interdisciplinary team.
• Provide leadership in case management/utilization concepts and education.
• Coordinate communication between providers, patients, families and outside agencies to ensure quality, cost-effective care.
