Qualifications

• Arizona RN license is required.
• Maintains BLS competency.
• CCM or ACM-RN preferred.
• One year of RN Case Management experience or fellowship within six months of hire.
• 3 years acute nursing and 1 year case management preferred.

Responsibilities

• Facilitate the patient care plan across the continuum of care.
• Lead in utilization management and care coordination.
• Educate on case management concepts and participate in committee work.
• Bridge communication among providers, patients, families, and outside agencies.
