Mayo Clinic is seeking an RN Case Manager to coordinate the patient plan of care across the continuum, ensuring utilization management, care coordination, and proper documentation. You will work within Mayo Clinic Nursing Professional Practice to drive quality outcomes and patient satisfaction.

The role emphasizes leadership, education on case management concepts, and collaboration with internal partners and outside agencies to deliver cost-effective care in a team-based environment.

Qualifications

• Arizona: Graduate of an accredited, or those in the candidacy process, baccalaureate nursing program.
• If graduated from a nursing program that was not accredited by ACEN, CCNE, and NLN CNEA, at least one year of RN experience in an applicable care setting is required.
• One year of RN Case Management experience or successful completion of the MCSHS RN Case Manager Fellowship within six months of hire required.

Responsibilities

• Coordinate patient plan of care across the continuum, ensuring utilization management and documentation.
• Lead and educate on case management concepts; collaborate with internal and outside partners.
• Bridge communication among providers, patients, families, and outside agencies to deliver cost-effective care.
