Mayo Clinic in Arizona is seeking a Registered Nurse Case Manager to coordinate patient care across the continuum and to ensure appropriate utilization management and care coordination. You will lead the team in documenting and communicating the patient plan, while upholding Mayo Clinic standards and ethics.

RN Case Manager responsibilities include education, collaboration with providers and outside agencies, and contributing to quality and cost-effective care delivery.

Qualifications

• One year of RN Case Management experience or completion of the MCSHS RN Case Manager Fellowship within six months of hire.
• 3 years of acute nursing preferred; 1 year of Case Management experience preferred.
• Certification (CCM or ACM-RN) preferred.
• Current RN license by applicable state requirements.
• Arizona - Maintains Basic Life Support (BLS) competency.

Responsabilités

• The RN Case Manager works within an interdisciplinary team to facilitate the patient plan of care across the continuum of care.
• Accountability for assessing, planning, implementing, evaluating and communicating the patient care plan progression.
• Provides leadership through education on case management concepts, committee work, research and community involvement.
• Bridges communication between providers, patients and families and outside agencies to ensure high-quality, cost-effective care.

Connaissances

RN Case Management experience

CCM/ACM-RN certification (preferred)

Current RN license

BLS competency

Formation

ACEN/CCNE/NLN CNEA accredited BSN or higher nursing program
