Job summary

Chapters Health System in Florida is seeking an experienced RN Case Manager to assess patient and family needs, coordinate plans of care with the IDT, and provide clinical, palliative and supportive care to keep patients at home as long as possible.

The role requires active RN licensure in Florida, at least one year of nursing experience (hospice or hospital preferred), and the ability to visit participants in their homes. A valid driver’s license and reliable transportation are needed.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
• Active BLS for healthcare professionals.
• Experience with EMR/EHR systems.
• Mobile driver - valid license and auto insurance.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient and family about disease, prevention, palliative care and safety practices.
