Job summary

Chapters Health is seeking a registered nurse (RN) Case Manager in Florida to assess patient and family needs, coordinate the care plan with the IDT, and provide clinical, palliative, and supportive care at home to keep patients in their homes as long as possible.

The role requires an active RN license in Florida, at least 1 year of nursing experience (hospice/hospital preferred), EMR/EHR proficiency, a valid driver’s license, and strong English communication skills; compensation is aligned with

Qualifications

Current RN license in Florida required.
Minimum 1 year nursing experience; hospice or hospital preferred.
PACE requires Alzheimer's training per Florida Dept of Elder Affairs.
Experience with EMR/EHR systems must be demonstrated.
Valid driver’s license and reliable transportation required.

Responsibilities

Provide direct nursing care as part of the IDT with a focus on home-based hospice.
Coordinate Plan of Care reflecting patient needs and evolving status.
Educate patient and family on disease, care planning, and safety.
Visit patients at home according to risk and schedule; document care delivery.
Communicate updates to IDT and physicians; SBAR communications used.
Supervise Home Health Aides as requested; participate in QA activities.
