Job summary
Chapters Health System is seeking an RN, Case Manager to assess patient/family needs, coordinate plans of care with the Interdisciplinary Team, and deliver clinical, palliative and supportive care in the home setting. Mileage reimbursement and sign-on bonuses are highlighted in the posting.
Position requires Florida nursing license, 1 year nursing experience preferred, and EMR/EHR proficiency, with an emphasis on compassionate, patient-centered care and strong communication with families and IDT
Qualifications
- Current RN license in the state of employment.
- Minimum of 1 year nursing experience; hospice or hospital preferred.
- Experience with EMR/EHR systems.
- Valid driver’s license and reliable transportation.
- Ability to visit patients at home and communicate in English.
- Active BLS for healthcare professionals.
Responsibilities
- Provides direct care to patients as part of IDT.
- Coordinates Plan of Care reflecting evolving needs.
- Educates patients and families on disease, palliation, safety.
- Home visits to assess safety, meds, and adherence.
- Documents changes and communicates with IDT.
- Responds to emergencies and supports referrals.
