Qualifications

• Current RN license in Florida.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience using EMR/EHR systems.
• Valid driver’s license and reliable transportation.
• Ability to visit participants in their homes for assessments.
• Excellent English communication (oral and written).
• BLS for healthcare professionals required.
• Ability to meet physical demands of the role.

Responsibilities

• Provides and manages direct patient/family care as part of the IDT.
• Coordinates Plan of Care reflecting patient needs and home setting.
• Educates patients/families on disease, care, and safety practices.
• Documents care and communicates with IDT using SBAR.
• Supports referrals and collaborates with hospitals and facilities.
