Job summary

Mayo Clinic is seeking a Registered Nurse Case Manager in Arizona to coordinate patient care across the continuum within an interdisciplinary team. You will ensure appropriate utilization management, care coordination, and timely documentation, following the Mayo Nursing Professional Practice Model.

The role emphasizes leadership through education, collaboration with providers and external agencies, and commitment to high-quality, cost-effective care.

Qualifications

• Graduate of an accredited nursing program; RN licensure as required by state.
• One year RN experience in an applicable care setting if program not ACEN/CCNE/NLN CNEA accredited.
• If graduation occurred more than two years ago, one year RN experience or Mayo Clinic LPN experience is required; RN Case Management experience or completion of the MCSHS RN Case Manager Fellowship within six months of hire is required.
• 3 years of acute nursing preferred; 1 year of Case Management preferred.
• Certification CCM or ACM-RN preferred; Basic Life Support (BLS) competency required.

Responsibilities

• Facilitate patient plan of care across the continuum with the interdisciplinary team.
• Lead in utilization management, care coordination, and clinical documentation.
• Educate and lead within the team; coordinate with external agencies to ensure high-quality, cost-effective care.
