Qualifications

• One year of RN Case Management experience or completion of MCSHS RN Case Manager Fellowship within six months of hire.
• 3 years of acute nursing preferred; 1 year of Case Management preferred.
• CCM or ACM-RN certification preferred.
• Current RN license by state requirements.
• Arizona BLS competency required; other licensure as needed by work area.

Responsibilities

• Coordinate care plans across the continuum of care with an interdisciplinary team.
• Ensure utilization management, care coordination, resource use, and clinical documentation are aligned with standards.
• Lead through education, committee work, and involvement in research and community partnerships.
• Bridge communication between providers, patients, families, and outside agencies to ensure high-quality, cost-effective care.
