Mayo Clinic in Buckeye, AZ seeks an experienced RN Case Manager to coordinate a patient’s plan of care across the continuum, ensuring appropriate utilization and timely documentation.

You will lead care coordination within an interdisciplinary team, provide education on case management concepts, and partner with providers, patients, and families to achieve high-quality, cost-effective outcomes.

Qualifications

• Arizona RN license required.
• If the nursing program was not ACEN/CCNE/NLN CNEA accredited, at least 1 year RN experience is required.
• 1 year RN Case Management experience or completion of MCSHS RN Case Manager Fellowship within 6 months of hire.
• 3 years acute nursing preferred; 1 year Case Management preferred.
• CCM or ACM-RN certification preferred.
• Maintain BLS competency.

Responsabilités

• Facilitate the patient plan of care across the continuum.
• Ensure utilization management, care coordination and timely clinical documentation.
• Provide leadership and education on case management concepts.
• Bridge communication between providers, patients, families and outside agencies to ensure quality, cost-effective care.
