Job summary

Mayo Clinic in Arizona seeks an RN Case Manager to coordinate care plans and optimize patient outcomes across the continuum of care. You will lead utilization management, care coordination, and clinical documentation, functioning within a professional practice model and collaborating with diverse internal and external partners.

The role requires an RN license, and one year of RN Case Management experience or completion of the fellowship within six months.

Qualifications

• One year of RN Case Management experience or completion of the MCSHS RN Case Manager Fellowship within six months of hire.
• 3 years of acute nursing preferred; 1 year of Case Management experience preferred.
• Current RN license by applicable state requirements.
• Arizona - Maintains Basic Life Support (BLS) competency.

Responsibilities

• Coordinate patient care plans within an interdisciplinary team.
• Lead in utilization management, care coordination, and documentation.
• Educate on case management concepts and participate in committees.
• Facilitate communication among providers, patients, families, and external agencies.
