Job summary

Mayo Clinic is seeking an RN Case Manager in Arizona to coordinate patient care across the continuum. You will work within an interdisciplinary team to ensure appropriate utilization and high-quality, cost-effective care, while leading education on case management concepts and collaborating with providers and families.

Required are an RN license, BLS, and either 1 year of Case Management or completion of the Mayo Case Manager Fellowship within six months of hire; CCM/ACM-RN is preferred.

Qualifications

• Arizona: Graduate of an accredited nursing program (ACEN/CCNE/NLN CNEA).
• If graduated from a non-accredited program, at least one year of RN experience in an applicable care setting is required.
• Current RN license by applicable state requirements.
• One year of RN Case Management experience or completion of the MCSHS RN Case Manager Fellowship within six months of hire.
• Certification (CCM or ACM-RN) preferred.
• Arizona - Maintains Basic Life Support (BLS) competency.

Responsibilities

• Coordinate patient plan of care within an interdisciplinary team.
• Ensure utilization management, care coordination, resource utilization, and clinical documentation.
• Provide leadership through education on case management concepts and committee work.
• Bridge communication between providers, patients, families, and external agencies to ensure high-quality cost-effective care.
