Job summary

Chapters Health System seeks a Per Diem RN to join its hospice care team in Florida. You will assess patient and family needs, coordinate care with the IDG, and deliver compassionate palliative support across facilities.

You will work with EMR/EHR systems, support timely admissions, documentation, and discharge planning while maintaining safety and compliance standards. Strong English communication and emotional resilience are essential for this role.

Qualifications

• Current RN license in the state where employed.
• Active BLS for healthcare professionals.
• Minimum one year nursing experience in hospice or hospital settings preferred.
• Experience using EMR/EHR systems.
• Valid driver’s license and reliable transportation per policy.
• Able to communicate effectively in English (verbal and written).
• Flexible schedule to meet Hospice House needs.
• Ability to safely lift patients and equipment and meet physical demands.
• Some locations require Alzheimer’s training completion per Florida guidance.

Responsibilities

• Completes clinical admissions for all services and explains offerings to patients/families empathetically.
• Initiates Plan of Care with the IDG and coordinates with physicians and facilities.
• Performs skilled nursing interventions to alleviate symptoms and comfort patients.
• Obtains physician orders and ensures timely documentation per policy.
• Communicates with Admissions and ensures smooth transitions and timely follow-ups.
• Provides education to patients, families, and care teams about disease and care needs.
• Maintains compliance with regulations and safety practices; participates in discharge planning.
