Job summary

Chapters Health System is seeking a Per Diem Registered Nurse to assess patients, coordinate care with the IDG, and provide compassionate palliative and supportive nursing care in hospice settings across Florida.

The role requires Florida RN licensure, BLS certification, and experience in hospice or hospital environments. A valid driver’s license and reliable transportation are needed for home visits, with a flexible schedule to meet patient needs.

Qualifications

• Current RN license in Florida.
• Active BLS certification from AHA or Red Cross.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provides and manages direct patient care as part of the Interdisciplinary Group (IDG).
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, palliative care, safety practices, and care planning.
• Reports changes in patient condition to IDG and other health professionals.
• Participates in admissions, initiates orders, and ensures timely documentation.
