Responsibilities
- Provide direct patient care as part of the Interdisciplinary Group (IDG).
- Coordinate Plan of Care to reflect evolving patient needs.
- Educate patients and families on disease process and palliative options.
- Report changes to IDG and physicians in a timely manner.
- Initiate skilled nursing interventions to alleviate symptoms and discomfort.
- Participate in performance improvement activities and maintain documentation.
