Chapters Health System is seeking a Per Diem RN to join our hospice team in Florida. You will assess patient and family needs, coordinate care with the Interdisciplinary Group (IDG), and provide palliative and supportive care at the patient’s location.

The role requires an active RN license, BLS, and experience in hospice or hospital settings; travel within the service area with reliable transportation is expected, and you must be able to lift and maneuver equipment as needed.

Qualifications

Current RN license for the state of employment.
Active BLS certification for healthcare professionals.
Minimum 1 year nursing experience in hospice or hospital setting preferred.
Experience with EMR/EHR systems.
Valid driver’s license and auto insurance; reliable transportation.

Responsabilités

Provides direct care to patients and families as part of the Interdisciplinary Group (IDG).
Coordinates the Plan of Care reflecting patient’s evolving needs.
Educates patient and family on disease process, palliative care, safety, and caregiving.
Reports changes in patient condition to IDG and participates in admissions decisions.
Initiates nursing interventions to prevent complications and alleviate symptoms.
Obtains physician orders and documents per policy.
Communicates with physicians, staff, patients and families; uses SBAR.
Supervises care provided by LPNs and contract nurses.
May be required to report to work during emergencies.
