Qualifikationen

• Current license as RN in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year of inpatient nursing or hospice experience (external candidates); for internal applicants, 6 months of LPN with hospice/home health within affiliate.
• Experience with an EMR/EHR system.
• Valid driver’s license and automobile insurance per Company policy.

Aufgaben

• Provides and manages direct care to patients and families as part of IDG, with appropriate nursing interventions and follow-up.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient and family about disease process, palliative care, safety, and comfort.
