Job summary

Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals (AHA or Red Cross).
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• EMR/EHR experience.
• Valid driver’s license and auto insurance per policy.
• Reliable transportation to meet visit schedule.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG.
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family on disease, palliation, and safety practices.
• Reports changes in patient condition to IDG and health professionals.
• Assists with admissions and discharge planning and documentation.
• Performs other duties as assigned.
