Job summary

Chapters Health System is seeking a Per Diem Registered Nurse to provide comprehensive hospice and palliative care within the patient/family unit. You will assess needs, coordinate care with the IDG, educate families, and document clinical findings as part of a dedicated care team.

The role requires an active Florida RN license, BLS certification, and experience in hospice or hospital settings. Strong communication, transportation, and flexibility are essential for this position.

Qualifications

Current RN license in the state of employment.
Active BLS for healthcare professionals (AHA or Red Cross).
Minimum 1 year nursing experience in hospice or hospital setting preferred.
Experience using EMR/EHR systems.

Responsibilities

Provides direct care to patients and families as part of IDG.
Coordinates individualized Plan of Care reflecting evolving needs.
Educates patients and families about disease, palliative interventions, safety.
Reports changes in condition to IDG and other health professionals.
Assists with hospice referrals/admissions for level of care.
Attends patient deaths and prepares body for transport.
Provides bereavement support with psychosocial staff.
Communicates with physicians, staff, patients and families; supports multidisciplinary input.
Supervises nursing team members as needed.
May be required to report to work at alternate locations during emergencies.
