Job summary

Chapters Health System in Florida is seeking a Per Diem Registered Nurse to support hospice patients and families with compassionate, medically integrated care. The RN will assess needs, coordinate care plans with the IDG, and ensure smooth communication across care teams.

Responsibilities include delivering direct patient care, documenting accurately, and maintaining compliance with regulatory standards. Flexibility for a variable schedule and travel within service areas is required.

Qualifications

• Current RN license in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year nursing experience in hospice or hospital setting preferred.
• Experience using an EMR/EHR system.
• Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to manage emotional stress of caring for terminally ill patients and families.
• Ability to physically access patient rooms and navigate various environments.
• Ability to perform essential functions including lifting and mobility tasks.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG, with psychosocial, spiritual, cultural, physical and biological components.
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, palliative interventions, care giving and safety.
• Reports changes in patient condition to IDG or other health professionals.
• Initiates skilled nursing interventions to prevent complications and maximize comfort.
• Maintains documentation per Company policy.
• Represents the Company at assigned facilities and facilitates referrals to service lines.
• Communicates with Admissions to ensure timely follow-up and prepares for next assignment.
