Qualifications

• Current Florida RN license or eligible multi-state license.
• Minimum 1 year inpatient/hospice experience preferred; LPN with affiliate hospice acceptable for internal applicants.
• Experience with EMR/EHR systems.
• Ability to communicate effectively in English (verbal and written).
• BLS for healthcare professionals on file.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG.
• Coordinates the Plan of Care reflecting patient needs.
• Educates on disease process, palliative care, safety, and caregiving.
• Communicates changes to IDG and other health professionals.
• Attends patient deaths and prepares body for transport.
• Performs other duties as assigned.
