Job summary

Chapters Health System in Florida is seeking a Per Diem RN to provide compassionate hospice and palliative care to patients and families within our care communities. You will work with the IDG to assess needs, coordinate care, and support timely, quality discharge planning.

This role requires an active RN license in Florida, BLS, EMR experience, a valid driver’s license, and a flexible schedule to cover patient visits.

Qualifications

• Active RN license in the state of Florida.
• BLS certification for healthcare professionals.
• Minimum 1 year nursing experience in hospice or hospital settings.
• Experience working with EMR/EHR systems.
• Valid driver’s license and auto insurance; reliable transportation.

Responsibilities

• Provides direct care to patients and families as part of the IDG.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patient and family about disease, palliative care and safety.
• Reports changes in patient condition to IDG and coordinates admissions.
• Initiates interventions to prevent complications and maximize comfort.
• Maintains clear documentation and communicates with physicians and staff.
