Job summary
Chapters Health System in Florida is seeking a Per Diem Registered Nurse to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Group, and deliver compassionate palliative care in the Hospice setting. You will work per diem, with a Florida RN license, BLS, EMR experience, a valid driver’s license, and the ability to meet physical demands.
Strong communication in English and flexible scheduling are essential to support terminally ill patients and families
Qualifications
- Current RN license in Florida.
- Active BLS for healthcare professionals.
- Minimum 1 year nursing experience in hospice or hospital settings.
- Experience using EMR/EHR systems.
- Valid driver’s license and automobile insurance.
- Strong English communication, verbal and written.
- Flexible schedule to meet hospice needs.
- Ability to perform essential functions with/without accommodation.
Responsibilities
- Provides and manages direct care to patients and families as part of the IDG.
- Coordinates the Plan of Care with the interdisciplinary group.
- Educates patient/family on disease process, palliative care, safety.
- Reports changes in condition to IDG and physicians.
- Documents consistently per policy and prepares for discharge.
