Mercy is seeking a Care Manager to join the Care Management team. The role focuses on discharge planning from admission to discharge in collaboration with the healthcare team and may address complex psychosocial needs to support safe, efficient patient transitions.

Responsibilities include coordinating throughput, ensuring adherence to Mercy standards, and leveraging strong communication and assessment skills. Shift details indicate Saturday through Tuesday, 10 hours per day.

Qualifications

• Graduate of an accredited School of Nursing.
• Current License in the state of employment.
• Bachelor's Degree of Science in Nursing (BSN) required.

Responsabilités

• Facilitates discharge planning from admission to discharge.
• Collaborates with the healthcare team to coordinate patient throughput.
• Intervenes with patients with complex or high-risk psychosocial needs as assigned.
