Mercy is seeking a Care Manager to support discharge planning from admission to discharge in coordination with the multidisciplinary care team. The role may involve intervening with patients who have complex psychosocial needs, aligning with Mercy's mission, values, and service standards.

As part of the Care Management team, you will collaborate with clinicians to ensure smooth patient throughput, accurate assessments, and appropriate care transitions within the hospital setting.

Qualifications

• RN license in the state of employment required.
• Bachelor of Science in Nursing required.
• 2-3 years acute care hospital experience preferred.
• Care Management or Utilization Management experience preferred.

Responsabilités

• Facilitate discharge planning from admission to discharge in collaboration with the health care team.
• Intervene with patients with complex or high-risk psychosocial needs as assigned.
• Perform duties in alignment with Mercy mission, values and service standards.
