Job summary

Mercy is seeking a Care Manager to join the Care Management team to facilitate discharge planning from admission to discharge in collaboration with the healthcare team. The role may involve intervening with patients with complex or high-risk psychosocial needs, in alignment with Mercy's mission and Service Standards.
