Mercy is seeking a Care Manager to coordinate discharge planning from admission to discharge in partnership with the care team. The role includes managing complex psychosocial needs, ensuring patient safety and quality care, and aligning with care guidelines.

The position emphasizes collaboration, strong communication, and advanced computer skills to support InterQual/Milliman guidelines. The shift is Sat-Tue, 10 hours per day, at Mercy facilities.

Qualificações

• Graduate of an accredited School of Nursing.
• Current nursing license in the state of employment.
• BSN required.

Responsabilidades

• Facilitates the discharge planning process from admission to discharge in collaboration with the healthcare team.
• Intervenes with patients who have complex or high-risk psychosocial needs as assigned.
• Performs duties in a manner consistent with Mercy mission, values, and Service Standards.
