Job summary

Mercy seeks a Care Manager to support discharge planning from admission to discharge in collaboration with the healthcare team. You may intervene with high-risk psychosocial needs and provide patient-centered care in alignment with Mercy’s mission and values.

As part of the Care Management team, you’ll ensure effective transition planning, exemplify strong communication, and utilize clinical judgment to optimize patient outcomes and coordination across departments.

Qualifications

• Strong communication skills (verbal and written).
• Discharge planning and care coordination experience preferred.
• Nursing license in state of employment required.

Responsibilities

• Facilitate discharge planning from admission to discharge.
• Intervene with patients with complex or high-risk psychosocial needs as assigned.
• Support care management operations aligned with Mercy standards.
