Qualifications

• Registered Nurse with BSN and current state license.
• Must be a graduate of an accredited School of Nursing.
• Knowledge of InterQual or Milliman Care Guidelines preferred.

Responsabilités

• Facilitate discharge planning from admission to discharge with the healthcare team.
• Intervene for patients with complex or high-risk psychosocial needs as assigned.
• Collaborate across care management to ensure quality patient care and safety.
