The role emphasizes strong communication, critical thinking, and the use of care guidelines to support neonatal to geriatric patients, with a 10-hour shift schedule from Saturday through Tuesday.

Responsabilités

• Facilitate discharge planning from admission to discharge in collaboration with the care team.
• intervene with patients with complex or high-risk psychosocial needs as assigned.
• Coordinate patient throughput and ensure quality patient care during transitions.
• Use InterQual or Milliman Care Guidelines knowledge to support care decisions.
