Qualifications

• RN with BSN required and licensed to practice in Missouri.

• Experience in care management or utilization management preferred.

Responsibilities

• Facilitate discharge planning from admission to discharge.
• Collaborate with healthcare team to coordinate patient throughput.
• Intervene with patients with complex psychosocial needs.
