Mercy is seeking a Care Manager to facilitate discharge planning and coordinate care across the inpatient team. The role supports smooth patient transitions from admission to discharge and requires collaboration with physicians, nurses, and social work staff.

The position emphasizes strong communication, assessment, and time management skills, with attention to Mercy's mission, values, and service standards. Shifts run Saturday through Tuesday, 10 hours per day.

Qualifications

• Graduate of an accredited School of Nursing, required.
• Current License in the state of employment, required
• BSN minimum requirement

Responsibilities

• Facilitate discharge planning from admission to discharge in collaboration with the healthcare team.
• Intervene with patients with complex or high-risk psychosocial needs as assigned.
• Perform duties consistent with Mercy mission, values, and Service Standards.
