Job summary

Mercy is seeking a Care Manager to coordinate discharge planning from admission to discharge in collaboration with the healthcare team. The role may intervene with patients with complex or high‑risk psychosocial needs as assigned.

Per the Mercy mission, the Care Manager ensures quality patient care and safety, while aligning with value-based metrics and best practices. Strong communication, assessment, and time management are essential, along with advanced computer skills.

Qualifications

• Graduate of an accredited School of Nursing.
• Current nursing license in state of employment.
• Bachelor of Science in Nursing (BSN) degree.
• 2–3 years acute care hospital experience preferred.
• BLS (CPR) certification at hire date or within 90 days.
• Certification in Case Management preferred.
• Knowledge of InterQual or Milliman Care Guidelines preferred.

Responsibilities

• Facilitates discharge planning from admission to discharge in collaboration with the healthcare team.
• Intervenes with patients with complex or high‑risk psychosocial needs as assigned.
• Works within Mercy Care Management team to ensure quality patient care, safety and efficiency.
