Job summary

Bon Secours is seeking an RN Care Manager to coordinate care for patients and support safe transitions across the continuum. The role uses a collaborative process to assess, plan, implement, and evaluate care, including social determinants affecting wellbeing.

Responsibilities include prioritizing patients in need of care management, documenting plans, and ensuring transition planning with the healthcare team. A BSN is preferred; licensure and ACM/CCM/ANCC certifications are valued.

Qualifications

• Bachelor of Science in Nursing (preferred)
• RN license in state where working
• ACM/CCM/ANCC certifications preferred
• 1 year of clinical experience required
• 3 years in acute care preferred
• Ambulatory or post-acute care coordination preferred

Responsibilities

• Identifies and prioritizes patients in need of care management services using holistic factors.
• Plans with patients and team to maximize health outcomes and cost-effectiveness.
• Maintains clear, concise, and timely documentation in patient records.
