Bon Secours Mercy Health seeks an RN Care Manager to coordinate care, plan transitions, and promote efficient, safe patient flow at Fairfield Hospital. The role involves holistic assessment, collaboration with interprofessional teams, and ensuring documentation reflects patient goals and care plans.

Minimum BSN, active RN license in Ohio, and BLS are required. Prior acute care or post-acute coordination experience is preferred. Competitive benefits and full-time schedule offered.

Qualifications

• BSN required and RN licensure in the state in which they work or compact
• BLS Basic Life Support – American Heart Association required
• RN license in state or compact required
• Accredited Case Manager Certification (ACM) or CCM/ANCC Certification preferred

Responsabilités

• Identifies and prioritizes patients in need of care management services using a holistic approach and plans with patients and care team.
• Maintains concise documentation in the patient record and coordinates level-of-care transitions.
• Ensures transition of care planning aligns with patient goals and involves family or caregivers as appropriate.
• Supports care planning, discharge planning, and readmission prevention through standardized processes.
