Job summary

McLaren Health Care is seeking an RN Care Manager to coordinate patient care and ensure smooth transitions across the continuum. You will work with physicians, nursing staff, social services and post-acute providers to optimize discharge planning and resource use.

Requirements include state RN licensure, a BSN or progress toward one within five years, and at least three years of acute hospital experience. Preferred: case management certification and BLS. Schedule: Full-time, days.

Qualifications

• State RN licensure required.
• BSN or pursuing degree with 5-year plan.
• Minimum three years of acute hospital care experience.

Responsibilities

• Perform care coordination assessments within 24 hours of admission.
• Collaborate with social work and other disciplines for safe transitions.
• Assess patient/family needs to reduce barriers and plan discharge.
• Review DRG/LOS and identify discharge planning needs with providers.
• Document assessments, plans, interventions, and reassessments in EMR.
