Bon Secours Mercy Health is seeking an RN Care Manager for St. Francis Downtown to coordinate care and support safe, timely transitions across the continuum. You will screen, assess, plan, implement and evaluate options to meet health needs, including social determinants, ensuring appropriate resources are available.

The role requires RN licensure, BLS, and experience in acute care; onboarding will emphasize collaboration with the healthcare team to optimize patient outcomes and transitions.

Formación

• Bachelor of Science in Nursing (preferred for BSMH, required for RSFH)
• RN License in state of practice or compact license
• ACM/ CCM/ ANCC certification preferred

Responsabilidades

• Identifies and prioritizes patients in need of care management using holistic factors.
• Plans with patient, caregivers and healthcare team to maximize outcomes.
• Completes documentation reflecting plan of care and transitions.
• Follow standardized practices for Advance Care Planning, Length of Stay, readmission prevention.
• Promotes proactive care and removes barriers to timely testing and treatment.
