Job summary

Bon Secours is seeking an RN Care Manager for St. Francis Downtown. The role coordinates care for patients, ensuring safe transitions across the continuum and collaborating with the care team to plan, implement, and evaluate services addressing health and social determinants.

Responsibilities include identifying patients in need, maintaining documentation, coordinating post-acute services, and promoting timely transitions while adhering to care plans and organizational standards.

Qualifications

• 1 year of experience in a clinical setting.
• 3 years of experience in an acute care clinical setting preferred.
• Ambulatory or post-acute care coordination experience preferred.

Responsibilities

• Identify and prioritize patients needing care management.
• Complete documentation in patient records and ensure smooth handovers.
• Address post-hospital care needs and coordinate resources.
