Bon Secours Health System in Southampton Medical Center is seeking an RN Care Manager to coordinate care across the continuum, ensuring safe, timely transitions and appropriate resources for patients.

This PRN position requires a nursing license and at least 1 year in clinical care, with preferred acute care and case management experience; benefits include competitive pay and flexible scheduling.

Competenze

• Bachelor’s Degree in Nursing is preferred; Associates in Nursing degree required.
• Active RN licensure in the state or compact license.
• ACM/CCM/ANCC Nursing Case Management board certification preferred.

Mansioni

• Identifies and prioritizes patients in need of care management services using a holistic approach.
• Completes documentation reflecting plan of care and handovers during transitions.
• Plans with patient, caregivers and healthcare team to maximize outcomes and cost-effectiveness.
• Follow standardized processes for Advance Care Planning, Length of Stay management and readmission prevention.
• Promotes proactive care and removes barriers to timely testing and treatment.
